Who runs Forma
DEN publishes Forma, a fitness and wellbeing journal. Contact danielipsky@gmail.com about privacy, access or deletion requests. Forma is intended for ages 13+; under-18 users should involve a parent or guardian. Do not use Forma to store medical records or highly sensitive personal information.
Information used to run your journal
Sign-in is provided by ChatGPT through Sites. Forma uses the authenticated account identifier to keep each person’s journal separate. Meals, portions, optional nutrition estimates and notes, meal plans, workout schedules and results, walking distance and duration, and preferences are stored in the hosted database. Approved daily AI ideas may also be cached there.
This information provides the journal, cross-device access, scheduling and the optional suggestions you choose. Forma does not sell journal data or use it for advertising. The application includes no advertising or analytics trackers. The hosting and identity services operate their own infrastructure and security logs under their applicable policies.
Photos and location
Photos are previewed in the current page only. Photo recognition is not connected. Forma does not upload or save those photos. If you start a walk and grant location permission, the browser processes location fixes to estimate distance while Forma is visible. Coordinates and route history are not saved or sent to the server; only the distance and duration you finish and save enter your journal. Camera and location access are optional and do not start automatically.
Optional AI and Groq
With permission for each message, Forma sends that message and up to six recent messages from the same section to Groq. Anything you type into a message, including personal information, is part of that request. The optional activity checkbox adds a server-built seven-day summary of sessions, effort, walking and approved preferences; Food can add today’s meal-entry count and types. Mental-health and Settings chats do not include this activity context.
Daily ideas have a separate opt-in covering your place, time, focus, available equipment, selected movements and recent activity summary. Raw journal notes, photos, GPS coordinates and custom session names are not included in that summary. Chats stay in the open page and clear on refresh or New conversation. Forma does not store chat messages or replies in your journal.
Groq processes requests under its data-handling policy, which may include limited reliability and abuse-monitoring retention. Turning AI off stops future optional requests; it does not undo a request already sent. Do not send identifying or sensitive information.
Storage and retention
Your journal remains until you delete its contents in Settings. In-progress workout steps are held in this browser tab, scoped to the account, for up to 24 hours. Usage-protection counters and time windows are stored separately and remain after journal deletion to prevent abuse. These counters contain no journal notes or photos. Hosting-provider operational backups and logs follow the provider’s retention practices; deletion from the active journal is not a promise of immediate removal from every backup.
Forma’s installed web app caches only a generic offline page. It does not cache journal API responses, chats or sign-in pages. Forma does not claim end-to-end encryption or medical-record security. On shared devices, sign out and close the page.
Your controls
In Settings, export journal records as JSON, turn off daily AI, delete journal records and preferences, or sign out. Journal deletion also removes cached daily ideas. It does not delete your ChatGPT account, downloaded exports, service counters or data already processed by a provider. Contact DEN for an access, correction or deletion request that the controls do not cover. We may need to verify that a request concerns your own account.
No payment details, birth date or contacts are requested by Forma. Browser permissions can be revoked in your browser or device settings. External recipe and guidance links have their own privacy practices. Material changes to these practices will be reflected here with an updated date.
